Physical Examinations

A Preparticipation Physical Examination is required after April 15th but prior to the first practice of the activity your child plans to participate in for the 2022-23 school year. Students wishing to participate in an athletic activity for the 2022-23 school year need to have the Medical Eligibility Form portion from the NDHSAA Preparticipation Physical Examination Form on file with the school.

 After the physical exam, the Medical Eligibility portion of the Preparticipation Physical Examination Form can be filed with the school by uploading the form during registration or by turning in a hard copy to the JH Office. Until we have this form on file, your child will not be eligible to practice.
